There is NO CHARGE merely for submitting a preliminary request or physician referral for preliminary review.
Step 1 — Complete the Preliminary Request
If you believe that you have a medical condition qualifying for treatment under the Texas Compassionate Use Program, begin by completing the secure Preliminary Patient Request.
At this stage, we ask only for the information necessary to make an initial determination of whether proceeding further would be appropriate.
You should not send extensive medical records or detailed confidential medical information through the general website contact form or ordinary email.

Step 2 — A Physician Referral Is Required
Dr. Rivers would appreciate a referral from a physician currently involved in your medical care. A Veterans Administration Disability letter will suffice to document validation of a qualifying condition as long as the qualifying condition is referenced on the VA Disability document.

What Should the Physician Referral Include?
The referring physician may complete the practice’s standardized Physician Referral Form or provide an equivalent signed and dated Letter of Referral.
Supporting medical records may also be requested when necessary.

Physician-to-Physician Telephone Referral
When a timely referral is appropriate, a physician currently involved in the patient’s care may also speak directly with Dr. Rivers.
Before confidential medical information is discussed, the patient must have completed the appropriate HIPAA authorization through TexasCURT.com permitting the necessary physician-to-physician communication.

Step 3 — Dr. Rivers Reviews the Preliminary Information
Dr. Rivers personally reviews the preliminary patient information and physician referral.
He may also review supporting medical records or communicate with the referring or treating physician when additional information is requested.
QUALIFYING CONDITIONS FOR PARTICIPATION IN THE TEXAS COMPASSIONATE USE PROGRAM FOR LOW-THC CANNABIS
- Amyotrophic lateral sclerosis (ALS)
- Autism
- Cancer
- Crohn’s disease or other inflammatory bowel disease
- Chronic pain
- Epilepsy
- Incurable neurodegenerative disease
- Multiple sclerosis
- Post-traumatic stress disorder (PTSD)
- Seizure disorder
- Spasticity
- Traumatic brain injury
- Terminal Illnesses requiring hospice or palliative care
In addition, the Texas CUP allows medical conditions designated by the state’s Health and Human Services Commission as qualifying for low-THC cannabis, if the treatment is part of an approved research program.
